Client Intake Information

At Luna's Moon I care about yours and your babies health and wellbeing which is why it is important for you to complete our client intake form to tell us a little bit about yours and your baby's health and other details. 

*I cannot accept you and your baby into my classes without prior completion of the form, for safety reasons.  

I will only use the information  provided for the benefit and comfort of you and your baby to inform me of any possible adaptions to my usual class structure may need to be made for your comfort and enjoyment  /to accommodate any known health issues or allergies of yours and your baby's. 

To also learn the name of you and your baby so I can warmly  greet you and welcome you both into my class when you attend for the first time and thereafter.

NB: your personal data will be securely stored on a password protected file only I have access to and will not be shared with 3rd parties without your prior consent in any situation. Confidentiality of full names, health concerns or any other nature will always be obtained!
